Health Care Vs. The Lower Middle Class


For many Americans, health insurance is a medical necessity and without it, many Americans would be left with medical bills that are too expensive to pay. Most people would agree that without some form of health insurance, purchasing prescription drugs would not be possible. Many Americans, such as those with diabetes cannot survive without certain prescription medications.

The upper middle and upper class citizens are generally able to afford health care, often times without needing health insurance and lower class Americans are able to obtain health care through organizations such as Medicaid, which usually pay for over-all incurred medical expenses.

So who did we leave out of this mix of American society? The lower middle class Americans who “fall between the cracks” of obtaining the quality health care they need.

Why can’t many lower middle class Americans afford quality health care? To provide an adequate answer to this question, we first need to define the American class system in terms of socioeconomics and its relationship to health insurance and the general health care system.

The working class or middle class is defined as a group of people who make up about 33% (the majority) of the population and bring home an average yearly income of $30K to $50K per year. The upper middle class also averages $55K to about $155K annually.
Compare those figures to the upper class, which make up only 2% of the population and contribute $155K to billions per year. Total health care costs, including insurance premiums have gone up drastically since 2001 and many employers are “sticking” the additional associated health insurance costs to their employees.

While many working class Americans are presented with health insurance opportunities, (usually through employment) many cannot afford the high monthly and sometimes yearly premiums associated with a health insurance plan. Other working class citizens are able to afford the insurance premiums, but are unable to take advantage of a health care plan because such a large percentage of their monthly or yearly income is set aside to pay for health insurance. This leaves many Americans left with the dilemma of making a choice between basic necessities such as paying for a monthly mortgage note, groceries or health insurance.

In essence, American society has created and contributed to a highly flawed health care system that caters to the rich, cares for the poor, yet excludes the largest percentage of its population.   • Too much stress over too long a period can even lead to heart disease or a heart attack. Whilst this is an extreme example of how stress affects your health, you should make certain that you address your stress levels to minimize the likelihood of your heart having to tell you that you are too stressed.

Universal Health Care - Ethical Issues In Health Care Reform


Universal health care seems to be a hotly debated topic whenever health care reform in the United States is discussed. 

Those who maintain that health is an individual responsibility do not want a system that requires them to contribute tax dollars to support fellow citizens who do not act responsibly in protecting or promoting their own health. They argue that they want the freedom to choose their own physicians and treatments, and suggest that government cannot know what is best for them.  These people argue that preserving the current system with improvements to provide better insurance coverage for citizens who remain uninsured or under insured for their medical care needs is the only reform that is needed.

Those who believe health care is an individual right support a universal health care system with the argument that every citizen deserves to have access to the right care at the right time and that a government's responsibility is to protect its citizens, sometimes even from themselves.

Two opposing arguments arising from two opposing ideologies.  Both are good arguments but neither can be the supporting argument for implementing or denying universal health care.  The matter must be resolved through an ethical framework.

Examination of the ethical issues in health care reform would require consideration of much different arguments  than those already presented.   Ethical issues would center on the moral right.  Discussion would begin with not "What is best for me?" but rather  "How should we as a society be acting so that our actions are morally correct?"

Ethics refers to determining right and wrong in how humans relate to one another.  Ethical decision making for health care reform then would require human beings to act in consideration of our relationships to each other not our own individual interests.

Examination of some of the common ethical decision making theories can provide a foundation for a different perspective than one that is solely concerned  with individual rights and freedoms. 

Ethical decision making requires that specific questions be answered in order to decide on whether intended actions are good or morally correct.  Here are some questions that could be used in ethical decision making for health care reform.

  • What action will bring the most good to the most people?
  • What action in and of itself is a good act and helps us to fulfill our duties, obligations, and responsibilities to each other?
  • What action in and of itself shows caring and concern for all citizens?

As the answer to all  these questions, universal health care can always be considered the right thing to do.

The United States is in the most advantageous position there is when it comes to health care reform.  They are the only developed country without a national health care system in place for all citizens.  They have the opportunity to learn from the mistakes that have been made by all the other countries that have already gone down the universal health care road. They have an opportunity to design a system that can shine as a jewel in the crown of universal health care systems everywhere. 

However, all ethical decision making is structured around values.  In order for universal health care to be embraced by all citizens in the United States, they will first have to agree to the collective value of equity and fairness and embrace the goal of meeting their collective responsibility to each other while maintaining individual rights and freedoms. That may prove to be the most difficult obstacle of all.

There have been positive reports in the Oxford Journal of Medicine and the Federation of American Societies for Experimental Biology. These reports indicate that there may be many benefits.

Resveratrol is also an antioxidant, which helps the body remove free radicals and other disease causing products from the blood stream. 3: Muscular Endurance - the ability of your muscles to exert force or to perform repetitive movements over prolonged periods of time. This component can be tested by doing sit ups or push-ups

4: Flexibility - the ability to move our joints and muscles through their full range of motion. This can be seen when doing stretches or splits.

5: Body Composition - this refers to the ratio of lean muscle to fats in the body. A good fitness program should include each of these five components of health-related fitness. You should begin each workout session with 5 to 10 minutes of stretching and low intensity warm-ups. Cardio respiratory endurance can be improved by doing at least 20 minutes per session of aerobic activity. This can be in the form of jogging, bicycling, brisk walking, rowing, swimming or playing tennis or racquetball. Lifting weights whether free weights or strength machines will help you improve your muscular strength. Muscular endurance can be improved by doing push-ups sit ups or pull ups. Flexibility can be improved by doing 10 minutes of stretching exercises a couple times per week.

By including each of the first four components in your fitness program you will improve the fifth component by increasing the amount of lean muscle in your body and reducing the amount of fats.

Health, Safety & Environmental (hse) Management In Engineering Practice


COMPONENTS OF HSE MANAGEMENT
Health, Safety and Environmental management should be part of the engineering profession in a country for the purpose of
o duty of care
o economic reasons and
o legal reasons.

HSE management should therefore consider five broad phases:
* Specifications
* Design and implementation
* Installation and commissioning
* Operation and maintenance
* Changes after commissioning.
* Compliance with the standards requires four essential elements:
* Identification of safety functions required for the safe shutdown
* Assignment of a safety integrity level (SIL) for each safety function
* Use of the safety lifecycle for the engineering design and
* Verification of the SIL achieved for each safety function.

3.0 ENGINEERING CODE OF PRACTICE
The engineering code of practice takes into consideration the following:
* Public safety: Giving priority to the safety and well-being of the community and having regard to this principle in assessing obligations to the clients, employers and colleagues.
* Risk Management: Taking reasonable steps to minimize the risk of loss of lives, injuries or suffering.

4.0 SAFE ENGINEERING DESIGNS
Safety is a concern in virtually all engineering design processes. Engineers should understand safety in the context of engineering design and what it means to say that a design is safe against human injuries.

Current design methods prioritize economic considerations over environmental ones. In some cases, economic considerations also serve environmental goals. For instance, the minimization of materials used in a structure means resources are saved. If they are saved at the expense of the length of the operating life of a product, then, economic considerations conflict with environmental interests which demand that products be made as durable as possible because of the need to minimize resource usage and waste generation in the long term.

Safety is the antonym of risk. So, a design is safe to the extent that it reduces risk. Safe design aims at minimizing risk in the standard sense of this term.

A safe design is the combination of all those procedures and principles that are used by engineers to make designed objects safe against accidents leading to human death or injuries, long term health effects, damage to the environment or malfunctioning in general.

Several design strategies used to achieve safety in operations of potentially dangerous technology are:
* inherently safe design
* safety factors
* negative feedback (self-shutdown) and
* multiple independent safety barriers.

Probabilistic Risk Assessment (PRA) is the most common method of assessing safety but safe designs are used to reduce risks in the standard (probabilistic) sense but is inadequate. Safe design strategies are used to reduce estimated probabilities of injuries or reducing uncertainties not only risks. They are used to cope with hazards and eventualities that cannot be assigned meaningful probabilities.

5.0 DESIGN PRINCIPLES IN ENGINEERING
There are four (4) main design principles in Engineering practice.

(a) Inherently safe design:
This minimizes the inherent dangers in the process as far as possible. Potential hazards are excluded rather than enclosed or coped with. For instance, dangerous substances are replaced by less dangerous ones and fire proof materials are used rather than inflammable ones.

(b) Safety Factors
Construction should be strong enough to resist load and disturbances exceeding those that are intended. A common way to obtain such safety reserves is to employ explicitly chosen numerical safety factors are employed. If a safety factor of two (2) is employed when building a bridge, then the bridge is calculated to resist twice the maximal load to which it will be exposed to in practice.

(c) Negative feedback mechanisms
This is introduced to achieve a self-shutdown in case of device failure or when the operator looses control. Examples are safety valves that let out steam when the pressure is too high in a steam boiler and the dead man's hole that stops the train when the driver falls asleep. One of the most important safety measures in the nuclear industry is to ensure that reactors close down automatically in critical situations.

(d) Multiple Independent Safety Barriers
Safety barriers are arranged in chains, so that each barrier is independent of its predecessors (if the first fails, the second is still intact). The first barriers prevent accidents; the second barriers limit the consequences of an accident and rescue services as the last resort.

Safety factors and multiple safety barriers deal with uncertainties as well as risks. But currently, Probabilistic Risk Analysis (PRA) is used but does not deal with uncertainties. Probabilistic calculations can support but will not supplant the Engineers' ethically responsible judgment (environment, health and safety culture).

Safety engineering principles also include education of operators, maintenance of equipment and installations and incidence reporting are examples of safety practices of general importance.

6.0 HEALTH, SAFETY & ENVIRONMENTAL MANAGEMENT IN ENGINEERING PRACTICE
The Engineering profession is expected to be the harbinger of Health, Safety & Environmental management by virtue of the complexity of the output of the profession and their impacts on the lives of the general populace. How have we carried this along in our professional practices?

Seven (7) bad engineering practices have been identified:
* Believing that if something is not specifically stated, either "shall do" or "shall not do" in the standards, an engineer does not need to worry about it.
* Thinking that meeting the minimum requirements means the process is safe and complies with the standard.
* Ignoring the importance of good engineering practice.
* Designing systems that meet economic requirements but not safety protection requirements.
* Neglecting human factors (errors in calculations etc)
* Focusing on capital cost and not on lifecycle costs.
* Focusing only on the safety integrity level (SIL) and not on prevention.

Safety is an essential ethical requirement in engineering practice. Strategies for safe design are used not only to reduce estimated probabilities of injuries but also to cope with hazards and eventualities that cannot be assigned meaningful probabilities. Designers have an ethical responsibility to make constructions that are safe for future use. Safety is concerned with avoiding certain classes of events that are morally right to avoid.

In engineering design, safety consideration always includes safety against unintended human death or injuries that occur as a result of the unintended use of the designed object for:
* Prevention of damage to the environment
* Prevention of long term health effects

For example, if a bridge collapses, the engineers who designed it are held responsible.

Building designers and builders must obey construction safety in the use of Scaffolds, tool nets, tool boxes, mechanical lifts and manual lifts under safe procedures, use of personal protective equipments (PPEs) on sites (boots/helmets), clear passages and road-ways, construction tapes to cordon off work areas etc. Most engineers have neglected this aspect, thus, playing with the lives of the generality of the populace.

What engineers do have lasting influences on safety and define our level of Environment, Health and Safety culture.

7.0 ETHICAL OBLIGATIONS IN ENGINEERING PRACTICE
Ethical obligations are necessary in order for engineers to carry out their profession. Without the obligation of confidentiality, clients could not trust engineers with commercially sensitive information. Without these information, engineers could not do their jobs. The moral obligations of our profession can be understood as duties which are necessary.

There are five (5) fundamental values necessary for the ethical obligations:
* Protection of lives and safeguarding of people.
* Professionalism, integrity and competence
* Commitment to community/public well-being
* Sustainable management and care for the environment
* Sustaining engineering knowledge

8.0 ENGINEERING PRACTICE REQUIREMENTS
* Engineers shall hold paramount the Health, Safety and Environment/welfare of the public in the practice of their profession.
* Engineers shall practice only in their areas or field of competence, in a careful and diligent manner and in conformance with standards, laws, codes, rules and regulations applicable to engineering practice.
* Engineers shall examine the societal and environmental impact of their actions and projects, including the use and conservation of resources and energy in order to make informed recommendations and decisions..
* Engineers should declare their interests clearly.
* Engineers shall sign and take responsibility for all engineering works which they prepared or directly supervised. Engineers may sign works prepared by others only with their consent and after sufficient review and verification.
* Engineers shall act as faithful agents for their employers or clients and maintain confidentiality, avoid conflicts of interest whenever possible and disclose unavoidable conflicts.
* Engineers professional concerns must be made known to the client and the consequences of engineering decisions or judgments.
* Engineers should reject any public works, engineering decisions or practice that endanger the HSE of the public.
* Engineers shall commit to life-long learning, strive to advance the body of engineering knowledge and should encourage other engineers to do likewise.
* Engineers shall promote responsibility, commitment and ethics both in the education and practice phases of engineering. They should enhance society's awareness of engineer's responsibilities to the public and encourage the communication of these principles of ethical conduct among engineers.

9.0 HSE SUSTAINABILITY MANAGEMENT
This is about the long term survival of humanity. It recognizes that decisions made today must enable both those in the present as well as people of the foreseeable future to make effective choices about their quality of life.

Failure to identify risks to safety and the inability to address or control these risks can result in massive costs, both human and economic. The multidisciplinary nature of safety engineering means that a very broad array of professionals are actively involved in accident prevention or safety engineering.

A critical fault endangers or few people. A catastrophic fault endangers, harms or kills a significant number of people. Engineer's errors or inability to incorporate the HSE management in his practice spells catastrophic.

10.0 THE WAY FORWARD
Everyone must strengthen his or her understanding of HSE awareness by making safety a priority. Also, cost effective solutions in order to gain the biggest return on investment should be developed.

Engineers take early design of a system, analyze it to find what faults can occur and then propose safety requirements in design specifications upfront and changes to existing systems to make the system safer.

If significant safety problems are discovered late in the design process, correcting them can e very expensive. This type of error has the potential to waste large sums of money.

* At all times, take all reasonable care to ensure that your work and the consequences of your work cause no unacceptable risk to safety.
* Take all reasonable steps to make your management/client and those to whom they have a duty of care aware of the risks you identify.
* Make anyone overruling or neglecting your professional advice formally aware of the consequent risks.
* It is critical for engineers to maintain a deep and broad understanding of the many technical and professional practice issues that they will inevitably encounter in their role as employees of public owners. This is achieved through appropriate education, training, experience, license, professional engineering practice and continuing professional development.

11.0 CONCLUSION
The engineering practice like the construction industry is the agent of social and economic development, the barometer of economic activities and a very large employer of labour in Nigeria. It accounts for over 60% of the total capital investment. It is the largest employer of labour (think of all the electrical, mechanical, civil, chemical and computer jobs in industries).

Health, Safety and environment, which is concerned with life and property must be taken serious in this profession. Safety procedures are necessary to prevent accidents, diseases and harmful effects on the health of the public at large arising from the activities in the industry on site and its environs or off site.

Good HSE management is visible in a country through the quality of the professional ethics of the professionals, engineers inclusive and the level of her health values and the state of her environment, that is, her level of cleanliness (personal hygiene and public sanitation).

It can only be developed through personal commitment, willingness and self-sacrifice because of the long and short run benefits therein. HSE management habit starts with safety consciousness. Safety consciousness dwells with each and every one of us and should be taken along to our professional practices.

People should recognize that their health and well-being are related to the quality of their environment and should apply thoughtful principles to attempt to improve the quality of their environment.

As Engineers, we should lead others in being safety conscious at all times and refraining from doing anything that may result to accident. We should apply safety measures to all our daily activities and take our safety and that of others around us as our responsibilities especially in our practices.

Finally, as Engineers we should pursue sustainable Health, Safety & environmental management and make it part and parcel of our engineering practice today for the continued relevance of our profession tomorrow. It is only when this is done that our professional ethics will be meaningful knowing fully well that the products of our professional practices have great impact on the lives of the entire citizens of this country.

Health, Safety & Environmental management habit is therefore not only necessary and but remains a vital ingredient of our professional ethics in engineering practice in Nigeria and elsewhere and this must be sustained always by all.

When starting with a catering business you will be required to have many licenses and certifications, and must adhere to Health Department regulations. Health and safety management is necessary in companies. Effective health policies and relevant training would decrease the losses suffered by companies due to occupational accidents and illnesses.

Health Insurance Vs Health Plans


There are four common reasons why most people do not have health insurance or health plans. These four reasons are: (1) You are self employed. (2) You are unemployed. (3) Your current job does not offer coverage. (4) You have been released from your parents' coverage. Whatever the reason may be, it is important for you to get covered.

While doing some research on healthcare, you may come across two different types of medical coverage. The first is "health insurance" which includes HMO's, PPO's, and traditional coverage. You may also come across something called discount "health plans". These healthcare types are a little different from one another and you should understand what they are all about before making a final decision on what kind of coverage you choose.

Health Insurance

When choosing a health insurance plan you are going to need to choose between three different sub categories: PPO's, HMO's, and Indemnity Plans. There is not much difference between a PPO (Preferred Provider Organization) and an HMO (Health Maintenance Organization). They are both options that your keep your premium costs down by only allowing you to see doctors or visit specialists that are members of their provider network. For example, when you see a member doctor, the doctor charges full price for the visit. You pay a co-pay amount of $5-$25 - depending on your policy, and the insurance company is billed for the rest. You must visit a doctor in the network or the insurance company may not pay their share of your visit.

Indemnity Plans are a little different. Also called "traditional insurance", they allow you to choose whatever doctor, hospital, or specialist you want, and your visit is covered. You pay your co-pay amount and your insurer will pay the rest. These plans may or may not be more expensive, so it is important for you to compare quotes between indemnity plans and PPO/HMO plans.

Discount Health Plans

These are great alternatives to PPO's, HMO's, and traditional insurance, and are becoming increasingly more popular. These plans offer discounts on doctor visits, emergency care, and prescriptions with participating providers in their network, and membership comes with affordable monthly payments. They offer financial relief in your time of need by preventing you from having to pay for 100% of your medical expenses on your own. For example if you have a 50% discount plan, get sick, and see a participating doctor that normally charges $70 for an office visit, being a discount plan member means you will only have to pay $35 for that visit. Many discount plans are offered by major medical insurance companies and were created for people on a budget.

Aluminum Health Risks - Real Or Myth?


For years we have been warned about the risk of aluminum in deodorants. A recent study reported by WebMD Health News indicates there may even be a link of aluminum in deodorant and other personal care products to breast cancer.

Dr. Philippa Darbre of the University of Reading in England has found evidence that aluminum-based active ingredients in deodorants can mimic estrogen in the body.

She goes on to state, "If the aluminum salts in antiperspirants enter the body and mimic estrogen, it stands to reason that constant exposure over many years may pose a risk."

Most commercial antiperspirants contain aluminum. For years the discussions about risk of aluminum centered around Alzheimer's. This degenerative form of dementia mostly affects persons over 65 years of age.

Some studies have found there are higher than normal concentrations of aluminum in the Alzheimer's brain cells. Other studies have either not been able to conclude this or are still questioning the results. However, it is known that large amounts of aluminum can contribute to nerve toxicity and therefore possibly Alzheimer's disease.

The question becomes, can small amounts over time cause harm or is the danger only with one large exposure?

It is easy to see how we can accumulate aluminum in our bodies, since deodorant isn't the only product that contains it. It is also found in many other sources including antacids, aluminum tubes used with some brands of toothpaste, baking powder, nondairy creamers, table salt, drinking water, and processed cheese. Aluminum cookware, of course, is also a consideration.

It makes sense that small exposures on a daily bases could result in accumulation in tissues over many years, just as Dr. Darbre reports.

Our bodies do not require aluminum. We also know what we rub on our skin, we do absorb into the body and bloodstream.

Although there are no studies to this date that conclusively point to aluminum as a major cause for breast cancer and Alzheimer's, the results are not all in yet.

If you are not a gambling person, you may want to choose your personal care products very carefully to avoid aluminum. Remember, cigarettes used to not be considered unhealthy. Today, we know better.

The cases against nationalizing health care are not necessarily based on reality, but could be. The likelihood that medical provision could have a negative effect on American is based on the exact nature of a specific bill that would be passed as law.

The most critical case against this type of government-sponsored help is in all likelihood how it could have a detrimental effect on our life politically-that America might give the government one more bit of themselves that they will never be able to get back. No matter what, American citizens are advised to keep watch.

Misconceptions and Concerns Many people were scared in our country this week, and probably rightfully so. People both for and against the nationalization of health care spoke up and expressed their concerns, but of course this is a very personal matter.

The most personal issue of all that was brought up this week was that pertaining senior citizens. The fear was that the American people would be required to "pull the plug on Grandma" as one news source said. Another news source had mentioned, however, that this was false.

There would be this option but that seniors would not be required to use this service. This of course still seems to make many American people nervous. Therefore, they are continuing to speak up about this issue any chance they can get.

Health - Carbonated Drinks Can Damage Our Health


Last week I wrote a health article on how chemicals we use everyday around our homes and offices can be detrimental to our health, and very damaging to unborn and young children.

This week there has been even more worrying health information given about the additive E211 in soft drinks, especially at this time of the year when some children drink vast quantities. Parents have been warned that they should limit the consumption of their children's soft drinks due to health fears over the safety of a commonly used preservative E211, as laboratory tests have shown that it can switch off vital parts of DNA, causing serious damage to cells. Concern is also given to the fact that tests suggest this could also result in other degenerative diseases such as cirrhosis of the liver and Parkinson's disease.

E211 is found in many soft drinks including Fanta and Pepsi Max and many other brands of highly consumed carbonated drinks. Peter Piper, a molecular biologist expert carried out the research into sodium benzoate or E211 at Sheffield University, where he found that it could damage the mitochrondia, an important area in DNA. However the Food Standards Agency and drink manufacturers insist that rigorous assessments are made before the approval and use of any of their additives, and it is deemed safe to health in the UK, EU and US for food use. The FSA says that consumers can continue to enjoy soft drinks knowing they are safe.

However, Peter Piper said that "these chemicals have the ability to cause severe damage to DNA in the mitochrondia to the point that they totally inactivate it, they knock it out altogether". The mitochrondia consumes oxygen which gives you energy, if this becomes damaged the cell starts to malfunction seriously. With this there is a whole range of diseases effecting your health now being linked - through damage to DNA, Parkinson's foremost, but also several other neuro-degenerative diseases and of course the whole general health process of ageing.

Sodium benzoate is regularly used to kill bacteria, yeast and fungi in soft drinks, fruit juices, jams and spreads, and salad dressings, as well as many other products. But it is in the soft drinks, which are consumed in higher quantities, and been used for decades by the carbonated drinks industry that is causing concern. Peter piper acknowledges the fact that sodium benzoate had passed the UK, European Union and US food safety tests, but claims that these tests were too old to be reliable, and that by the criteria of modern safety testing are inadequate for our health

Safety testing in other areas has moved forward in the past 50 years with new research, and so rigorous new tests should be conducted on E211 and other additives. Sodium benzoate is found naturally in some fruit such as apples, cranberries, prunes, greengages and also cinnamon and cloves and as you know these are good for your health. When sodium benzoate is mixed with vitamin C it forms a carcinogenic substance called benzene.

I believe that we should not stop drinking carbonated drinks altogether, but the consumption should be limited, and maybe production of carbonated drinks with a shorter shelf life, needing less additives would be a good step for manufacturers to take. This would probably bring the price of carbonated drinks more into line with healthier alternatives available, as at the moment they are the cheapest option in most supermarkets.
